Suicide deaths involving heavy alcohol use have increased significantly among women in recent years, according to a new study supported by the National Institute on Alcohol Abuse and Alcoholism (NIAAA). Previous research has shown that alcohol is a risk factor for suicidal behavior and that women have a higher risk than men do for suicide while intoxicated. And in the two decades leading up to 2018, suicide death rates in the United States increased, with the rate among women increasing faster than the rate among men. The observational studies addressing suicide rate among alcohol abusers without comparison group or self-harm without suicide intention were excluded. Induction of buprenorphine in the emergency room for individuals with OUD who present with opioid overdoses has been shown to decrease the risk for future overdose 250. Interestingly, buprenorphine has shown efficacy in treating depressive symptoms during the course of treatment of OUD 251, as well as in treatment-resistant depression 252,253,254. Additionally, buprenorphine has shown promise in reducing suicidal ideation 255, 256. Some case reports reported significant reduction in suicidal ideation with the start of buprenorphine treatment for OUD 257, 258. Even in individuals without OUD, Yovell et al. 259 found that a very low dose of buprenorphine (0.1–0.8 mg/day) significantly reduced suicidal ideation in 2 weeks, compared with placebo. Interestingly, prior studies provided evidence for the rapid antidepressant and anti-suicidal action of buprenorphine, which seemed to act within a week after the first administration 252, 254, 258, 260,261,262,263,264.

The new research examined the increase in suicide mortality among women in the context of data showing an increase in heavy alcohol use over time. The study included data from the National Violent Death Reporting System, in which 115,202 suicides—including 87,771 men and 27,431 women ages 18 and up—were reported between 2003 and 2018. Suicides among people who had a blood alcohol concentration (BAC) of 0.08 g/dL or greater were considered alcohol involved. We can have high confidence based on the current evidence that AUD increases the risk of suicide. Therefore, further research is very unlikely to change our confidence in the estimate of effect. This finding supports the alcohol cessation programs to reduce alcohol use among the general population.
